CO Limited License Surrender

In the past Colorado had the option to surrender a limited license within a few days of the draw results for a refund AND reinstatement of points. I’ve done it myself when I drew a bear tag but didn’t get my deer tag. Looking at their website it appears today is that deadline but I can’t find the proper form to fill out. The only form I can find you have to choose points or refund but says you can’t get both. Can anybody point me in the right direction, please.

What tag? link to the ONLINE surrender form is at the very bottom of your CPW email recieved telling you you drew. ChecK yoru spam folder if it did not land in your inbox.

I also found the version you print and fill out and mail in with a basic google,search but the above was intended for the purpose you state (refund plus points).
