My google fu has been weak and my organizational skills weaker still, as I cannot find a or my copy of the Colorado BHS application form! Anyone that has a blank one and wouldn't mind emailing me a scan, please pm me.

I plan on calling DOW later to see if they can get one coming.

It has to be on their official form on paper. No copies.

They will send you one and when I needed one they were pretty quick as in I think I got it in Texas 2 days after I called them.

FWIW, CO DOW will be putting the official form on their website 3/25 for download. Not sure why they wait until that close to the deadline, but there it is.

They do that every year. I'm not sure what the point of waiting until the last week is, but it probably has to do with them preferring that folks use the original form.
