Need some help navigating cpw website

We’ll guys I’ve been planning and waiting for this 2nd season elk hunt for two years. I have to say it was stupidity that put and end to it. Sunday afternoon I climbed up a 19’ ladder stand and the strap at the top broke. I fell as far as it was tall and shattered my L2 and L3 vertebrae. So elk hunting and the rest is out for this year. I’ve got ten screws and two rods to repair it. I called cpw and they said I can return my tag with a medical exception for exchange. I’m just to drugged up to find the correct form could you guys help me out? Thanks in advance and be safe out there this season.

That’s the one you need. Hope you heal fast. Your lucky to be alive. That stuff really scares me with straps breaking on tree stands
