Not by an outfitter but by a Wyoming game warden. I had made sure to get a replacement copy before our trip out so I was good to go.

No. Only hunted twice with outfitters. Africa requires no previous experience or license. I did hunt with a waterfowl outfitter in SK a few years back. He took us down to get our non-resident licenses and we only had to produce valid licenses from our home jurisdictions. No hunters safety certs. I received my certification back in 1964. I haven't seen that card since I turned sixteen and it was no longer necessary to get licensed.
